Gentlemen;
There seems to be some doubt as to who I am, or if I signed a Company form w36-05 on 5-7-87 pertaining to policy N7232102B.
Please allow me to give brief history of myself to provide you with some material for your ongoing procrastic investigation.
I have been told on more than one occasion that I was born in or near Carter county Okla. on or about 24 April 1934 at a very young age.
Being half Choctaw and half white, and in the prevailing social conditions imposed on mixed Indians at that time, I was given an Indian name, with the understanding that at some later date I would be given a white name. This was usually done shortly before an Indian child was to enter school, and this was the case with me. My Indian name is Haddock Okla Chickalo (Try to pronounce that if you are non Choctaw) and the Dr. who delivered me probably could not spell it so he entered the words LIVE BOY on the application for a birth certificate.
At the tender age of five I was enrolled in Mt. Washington school, located somewhere north east of Ardmore, Okla. under the name of Carl Dean Adams.
There, the teachers endeavored to teach me a number of things, not the least difficult of which reading, writing, and speaking English. They were not totally successful in any of their efforts.
Due to mitigating circumstances, ( Hunger) my parents were forced to move a number of times while I was attending primary school. The chronological order is as follows; Mt. Washington, Deece, both in Carter County Okla. Brinkman, Ozark, White Flat, East View, and Granite, All located in Greer County Oklahoma.
While in high school I applied for a social security number and was issued XXXX-XX-5357.
I entered the U. S. Air force on the 15th. of July in 1953 and was then identified as AF 18-449-357. When I moved to Texas upon graduating from high school I applied for a Texas commercial operators license and was issued license &3968287, Later to become class A 803968287.
About six months before I was to be discharged from the Air Force I was told I would have to have a birth certificate before I could be released on the civilian population. I was stationed at Kindley Air Force Base in Bermuda so I wrote to The Bureau of Vital Statistics in Okla. City and requested a copy of my birth certificate and was finally informed that I did not exist.
I thought this over for a little while and said to myself ~Self They Wrong".
With my very limited literary ability and worse penmanship, I wrote back and listed all of the particulars pertaining to my birth, and admitted right up front that this was all hearsay on my part because I did not remember any of it.
I must have been convincing because within 45 more days, I received a birth certificate showing my date of birth as 4-24-34 and my parents as Frank and Velma Cheeves} Adams and my name as Live no middle name Boy. It was never clearly explained to me why, but this was not acceptable to the Air Force. After a lot more correspondence between Kindley Air Force Base and Okla. City, I was issued a birth certificate with my white name on it, Probably to get their paperwork level down to somewhere near bearable.
I was discharged from the Air Force on May 8, 1957 with a birth certificate.
Over the next 10 to 15 years I worked by the hour and attended school. My student number at the University of Houston was 75474. I later become an instructor there and my number was 54116. If my memory serves me correctly, I accumulated a total of 745 college hours before I entered the Civil Aviation industry. I since have earned a commercial pilot's license with single, multi-engine,' Instrument, Helicopter and D.C. 3 endorsements. I am also a flight -instructor and over the years I have added all of the endorsements on my pilots certificate to my instructor certificate. The number of my pilots license as well as my instructors certificate is 1874731.
While I was doing this, partly to keep myself in off the street and out of the pool halls, I was able to earn an airframe and power plant mechanics license, and have also been issued an Maint. Inspector Authorization. The number of all of these is 81991791.
I am also mildly involved in politics. I ran for and was elected County Chairman in 1980 and have been re-elected each time since. Some people have expressed a preference that I run for the County line, but so far they have not been really aggressive about that.
I just remembered that I am going to have to keep this letter short on account of my throat. Yes, my secretary had already threatened to cut it over some un-necessarily long letters I have written in the past.
If, in your ongoing procrastic investigation, you find that I do not exist, Please notify the I.R.S. If you find that I do exist, but am someone else, please inform me. If you find whoever I really am is:(A) younger (B) better looking (C) more affluent (D) any combination of the above, please call me collect ASAP.
